Yeah, you gotta check em now and then. They have a decieving number of somewhat hidden screws holding them together, and mine have come loose. Luckily not to the point anything fell off. But they do need to be tightenen occasionally i suppose. I just tightened mine real good in hopes it won't happen again. Like anything on a bike they can come loose and need occasional checking. Problem here is that the manufacturer probably didn't tighten them enough on the assembly line. They should replace it. After all, whats it cost them? But you know how that goes. they may well not do so. Smaller manufacturers like them are often tightwads even at the expense of losing a customer.
