Like before, on the form it will ask your for your screen name and add it to the card. You will get a full sheet of ten cards ready to print on Avery Business Card stock (or just print them and cut them out yourself).
If you are using Internet Explorer and you have problems with the PDF downloading, try switching to Firefox, it is a known issue that Internet Explorer and PDF Forms sometimes do not get along
