These are easy to get on Ebay for about $30. They are the same ones I have on my bike except a little shorter. You can use the single piece cap or the individual caps on them.

I made my own adapters too, I can't remember if I bought 2 stainless bolts 1/2" X 13 and just turned the other end down and threaded or bought a 5/8" stainless bolt and turned the whole thing down. The latter is more likely so I had plenty to grab in the chuck.

EDIT: Now that I look at them side by side in the post I think they are very close but not the same.

EDIT 2 : Now that I look again I think they are the same as mine just mounted on the wrong sides.
