Thanks for the pictures. The head of the nipple actually blew off so the spoke didn't "pull" through, it really fell out. Probably a bad batch of nipples, or more likely they were damaged during installation, this really should be a warranty claim. In any event you'll want to replace the whole lot. The spokes look fine to me, didn't break or bend, but the peace of mind replacing them gives is probably in order as well.
