Restroom cleanliness is a key indicator to me also. I figure if they can't keep that clean, I don't want to eat there. I once took a manager by the neck and stuffed his head reaaallll close to the urinal, so he could see and smell what I was complaining about.
Well, "HE STARTED IT!"... If I tell you your urinal is gross, don't stand 3 feet away and tell me "I don't see a problem!"
