A long shot, but check to see if the washer is on the left side. It shouldn't be. If it is use it on the right side. And check to see if it is between the spacer and the adjuster on the right hand side too.
The washer is a thick one. Almost as thick as a brick... er, jethro tull notwithstanding, as thick as the nut's flange.
