I have an odd-sized foot, to go with my odd-sized body, and had a pair of custom boots made by Wesco. Cost a bit--around $400--but they fit like a glove. After you order, they send you a fitting kit with detailed instructions for measuring a bunch of spots on your feet, ankles and calfs. They even include the cloth tape measure. You choose all the features you want, and the boots show up at your door about 3 or 4 months later. Okay, so it's not the best possible situation, but I got exactly what I wanted, great quality and they feel great.
