Tattoos will spread somewhat. However, if the artist knows what they are doing the spread will be minimal. The ones that become ink splotch were done too deep. They put the ink so deep into the skin layers it spreads. If the artist is good, it will hurt little, not bleed, heal well and not spread after years.
I have twelve. Mostly celtic knotwork that I drew myself. I have two with names. My son and my grandson. I have two that are words. One is in Cherokee the other is in ancient Etruscan. (which pre dates latin)
