First I would take the cash out option. then take my 150 million and

Pay off all my family debt.
I would open a multi line dealer ship ( Triumph, Aprilla, MV Agusta, and maybe one more exotic line.

I would buy the company I work for, it makes money hand over fist..

Maybe buy out many of the local ambulance compaines and combine them into one. Hire new people get rid of ones who i feel have no business being in a ambulance, and expand to a state wide system.

My yaxht of course, and a house here, a cabin in the mountains and a house in Jacksonville FL..
